This material was shipped to Fenwick Library Special Collections, George Mason University, Fairfax, VA on 23 October 2000. It is just one page, holding two tables or lists, labelled Mistakes of Commission and Mistakes of Commission, with an explanatory paragraph which reads: "Here are some of the most common types of mistakes that are made in systems work. The 'Mistakes of Commission" are things people do that they ought not to do. The "Mistakes of Omission" are things people ought to do that they do not do. I found copy in home boxes in 2012 and scanned it, had it in my database as "Mistakes of Commission, Mistakes of Omission" but I am changing to the GMU title which is "Mistakes of Omission & Commission". There is no actual title on the page, just the two lists. (Speaking of mistakes of commission, I scanned the same page twice, but won't go back to rescan to just one page, the digitized document is now 2 PDF pages on my computer.)
