Abstract:
The failure of a system is generally attributed to mechanical defect(s) and/or human error(s). However, there are other factors too. This paper presents the limitation of human communication, varied perception of optimality and discrepancies of behaviour between system and it components to be responsible for the unsatisfactory performance. The challenges to management to overc ome these elements are discussed.
